Players Membership Form

The Players Team Registration Form is to be used for players who were registered to teams last season, no photographs are required for this form, if a player moving from another team state which team in the appropriate column. this form may be emailed to your appropriate age group Registration Secretary (see below)
PLAYERS MEMBERSHIP REGISTRATION FORM is for new players who have never been registered to play football in the PJAL this form will require two passport size photographs with no hats and plain background
plus a copy of the players Birth Certificate or Passport or a document that can confirm the players date of birth.
