As per Rule 23H the appropriate form must be used when a mark of 60 or lower has been awarded.
 The form may only be completed and signed by the Club Secretary

Completed forms MUST be sent within 3 days of the match to the ERCFA's Ian McLane at: mclane59@hotmail.com

A copy should also be sent, on behalf of the RCERCFL, to Gary Edwards at: garye7671@gmail.com

Clubs failing to comply with this Rule shall be liable to be fined.
